Payments Prior To/On the Date of Service

Make sure to enter the account number provided to you by your Baylor Scott & White Surgicare Fort Worth patient representative.

If you are unable to pay the patient responsibility estimate in full prior to/on the date of service, please contact us directly: 817-334-5100. A Baylor Scott & White Surgicare Fort Worth patient representative can assist you in setting up a payment plan prior to/on the date of service.
